VERIFICATION REQUIREMENTS
Following are the verification documents required for 2012. You must bring copies of the following items to complete the registration process.
1. A minimum of three proofs of residency documents. Following are examples of accepted proofs of residency.
RESIDENCE ELIGIBILITY REQUIREMENTS
Residence shall be established and supported by documents from
THREE OR MORE of the following categories to determine
residency of such parent(s) or guardian:
1. Driver’s License
2. Voter’s Registration
3. School records
4. Welfare/child care records
5. Federal records
6. State records
7. Local (municipal) records
8. Support payment records
9. Homeowner or tenant records
10. Utility bills (i.e., gas, electric, water/sewer, phone, mobile
phone, heating, waste disposal)
11. Financial (loan, credit, investments, etc.) records
12. Insurance documents
13. Medical records
14. Military records
15. Internet, cable or satellite records
16. Vehicle records
Note: Example – Three utility bills (three items from No. 10 above)
constitute only ONE document.
2. A copy of the registered child’s birth certificate.
3. A volunteer check made out to SJALL for $200 and dated June 10, 2012. Each FAMILY is required to provide a volunteer check. At closing ceremonies volunteer checks for those families completing their required hours will be returned.
Tryouts for players interested in playing AA, AAA and Majors (ages 7-12) we be held on Saturday January 14th and Saturday January 21st. Players are required to attend both sessions. ***If your 7 year old is registered to play A ball, they are not required to attend the Tryout sessions. |
